To make a page, yes: you sign in with your EnderPhone account. It's free, and it works on any server. Anyone can view pages, and people can open yours from the phone's browser in game.
Yes. Links, widgets, templates, effects and analytics are all free. Premium, coming later, will be a one-time unlock for extras like custom fonts and video backgrounds.
You sign in with your EnderPhone account: your Minecraft name and the password you set on the phone's Account page. One name per account, and nobody can edit your page without it.
